چکیده
A 29-year-old woman was admitted with a 2-day history of fever, cough, hemoptysis, abdominal pain, and diarrhea. She had received a renal transplantation 4 months before and was receiving mycophenolate mofetil, tacrolimus, and prednisone. Two days after admission, she developed a petechial rash on the abdomen (Figure 1), paralytic ileus, and progressive respiratory distress requiring mechanical ventilation. The white blood cell count was 4,300, with 12% eosinophils, blood cultures were negative, serological tests for human immunodeficiency virus (HIV), human T-cell lymphotropic virus type 1 (HTLV-1), and cryptococcal antigen were negative, and plasma viral load for cytomegalovirus (CMV) was negative. The chest X-ray showed bilateral interstitial infiltrates (Figure 2). A bronchoalveolar lavage tested negative for bacteria, galactomannan antigen, mycobacteria, and fungi, but it disclosed rabditiform larvae of Strongyloides stercolaris (Figure 3), which were also found in the stools. She was started on ivermectin and broad antibiotic coverage but died of multiorgan failure 4 days later. Strongyloidiasis is a well-known complication of severe immunosupression. Conditions such as HIV and HTLV-1 infections, chronic use of steroids, chemotherapy, diabetes, hematologic malignancies, and malnutrition pre-dispose to the hyperinfection syndrome, which carries high mortality rates. Pre-transplant patients and donors in endemic areas should always be investigated for

Unusual manifestation of cutaneous Leishmaniasis in a renal transplant recipient
Cutaneous leishmaniasis may present as unusual manifestations in renal transplant patients receiving immunosuppressive therapy. This misleading presentation, may delay the diagnosis and treatment. Moreover special caution must be taken in renal transplant recipients because of possible interactions between antimony compounds and cyclosporine metabolites. We report a 45-year old man with 5 years...

Introduction: Myopathy and rhabdomyolysis are not common in children and, if not detected and do not treated it will be associated with high mortality and morbidity rate. The causes of rhabdomyolysis include hypokalemia, trauma, viral myositis, poisoning, rheumatoid diseases, and metabolic myopathies. Rhabdomyolysis treatment includes rapid supportive care and treatment of the underlying dise...
متن کاملFirst Report of Cryptococcus Albidus-Induced Disseminated Cryptococcosis in a Renal Transplant Recipient
Cryptococcus albidus, a non-neoformans species of the genus Cryptococcus, is generally regarded as a rare cause of disease. There have been only 14 previously reported cases in which this organism has been isolated as a pathogen, none of which occurred in a renal transplant recipient. A 23-year-old renal transplant recipient taking medication consisting of cyclosporine and prednisolone was admi...
متن کاملRupture of Renal Transplant
Background. Rupture of renal allograft is a rare and serious complication of transplantation that is usually attributed to acute rejection, acute tubular necrosis, or renal vein thrombosis. Case Presentation. LD, a 26-year-old male with established renal failure, underwent deceased donor transplantation using kidney from a 50-year-old donor with acute kidney injury (Cr 430 mmol/L). LD had a sto...
متن کاملUnusual Manifestations of Acute Cytomegalovirus Infection in Solid Organ Transplant Hosts: A Report of Two Cases
Cytomegalovirus (CMV) infection is a common cause of morbidity and mortality in immunocompromised hosts. Tissue-invasive CMV disease causing ulcerative skin disease or esophageal necrosis is rare. We herein describe two cases: a 47-year-old renal and pancreas transplant recipient who presented with skin ulcerations on his elbow and a 50-year-old renal transplant recipient who presented with acu...
